Premera has a specific form for Massage Therapy prescriptions. Please print off the form and take to your Primary Care Provider. The form may be faxed to our office and we then forward it onto Premera. This ensures a copy of your prescription is in our files.

Other Medical Insurance Companies, including Personal Injury and Worker’s Compensation require a medical referral/prescription for massage therapy.
