Medical Insurance Companies, including Personal Injury and Worker’s Compensation require a medical referral/prescription for massage therapy. Please print off the form and take to your Primary Care Provider. The form may be faxed to our office and we then forward it onto your insurance carrier. This ensures a copy of your prescription is in our files.

· Physician Prescription (required for all insurance clients)
· Private Insurance Information (Regence, Premera, Aetna, etc.)
· Personal Injury Insurance Information(Auto Accident or PIP)
· Worker’s Compensation Insurance Information (L&I)
· Initial Injury Form (to accompany PIP and L&I Forms)
